NEEDS ASSESSMENT: You’ll be connected with a Senior Care Advisor in our corporate offices for a free telephonic needs assessment and program overview. 

SENIOR CARE ADVISOR: Our Senior Care Advisor will determine if you meet the criteria to move forward on a live call with a Registered Nurse (RN). The Employee and Family Member/Policyholder are present to assess clinical, social and financial needs for community based resources.

NURSE VISIT: If recommended by our Senior Care Advisor, a home visit will be made by our Registered Nurse or Licensed Clinical Social Worker.
